Researchers have discovered a link between heart failure (HF) and a decline in people's mental processes and loss of grey matter in the brain.

A team of researchers at the Western Australia Institute for Health and Ageing found that patients with heart failure have a harder time with memory and reaction time than their peers. Researchers also found that the mental decline experienced by those patients made it more difficult for heart patients to remember to take certain medications or carry out medical instruction.

Study findings were published in the journal European Heart Health.

"Our results are consistent with the observation that people with HF have trouble adhering to complex self-care advice, and suggest that simpler approaches to self-management may be required," said the study authors.

Osvaldo Almeida, professor, and Director of Research at the Western Australia Institute for Health and Ageing in Pert, along with his colleagues, gave cognitive function tests to 35 patients with heart failure; 56 patients with ischaemic heart disease, which doesn't always accompany heart failure; and 64 health people without heart failur or ischaemic heart disease. Patients were also given MRI tests to assess difference in the volume of grey matter various regions of the brain.

Results showed that those with heart failure, had poorer immediate and long-term memory and reaction speeds than the healthy subjects. It was also noticed that heart failure is linked to changes in brain regions that are essential for certain cognitive functions.

"What we found in this study is that both IHD and HF are associated with a loss of cells in certain brain regions that are important for the modulation of emotions and mental activity, such as loss is more pronounced in people with HF, but can also be seen in people with IHD with HF," said Almeida, in a statement.

"Similarly, people with IHD and HF show subtle deficits in cognitive abilities compared with control without either IHD or HF, and again those deficits are more pronounced in people with HF. "

The areas of the brain that showed a lack of grey matter, are directly linked to reasoning, memory and planning.

"There is evidence that they optimize performance in complex tasks that require mental effort. Consequently, loss of brain cells in these regions may affect a person's performance in a number of different areas, such as memory, behavior modification, inhibition, both emotional and cognitive, and organization," said Almeida.

[Abstract]

Lung cancer is one of the common malignant tumors and it is a world-wide leading cause of human cancer death. In China, the incidence and mortality of lung cancer rank the first place in the city and the third in the countryside. Lack of the effective early diagnosis method is the main reason of the high mortality and poor prognosis. Theoretically, a molecular marker is the most sensitive indicator for early diagnosis of cancer. Therefore, screening for biomarkers with high sensitivity and fine specificity have become the key to early diagnosis for lung cancer.In this study, 2-DE(two-dimensional gel electrophoresis) combined with MS (mass Spectrometry) and SDS-PAGE (sodium dodecyl sulfate-polyacrylamide gel electrophoresis) combined with MS, were performed to screen the differential proteins from healthy subjects and stage 1 lung squamous carcinoma patient sera.First,2-DE and SDS-PAGE were used to separate proteins from healthy subjects and patient sera, respectively. The protein spots and bands were analyzed by the image analysis software and the differential protein spots and bands were identified by ESI-Q-TOF MS/MS (Electrospray ionization- quadrupole time of flight mass Spectrometry). And then,western blot and immunohistochemistry were used to determine the expression level of haptoglobin-2(HP-2) , one of the differential proteins identified by two technical routs, in the serum and tissue,respectively. The clinical significance as well as the cause of HP-2 regulation in serum were investigated.Through 2-DE in combination with MS,ten differential protein spots were detected and four proteins, including AIM,Transthyretin-related, Retinol Binding Protein and Hp-2, were identified.Through SDS-PAGE in combination with MS ,five differential protein bands were detected, and fifty proteins were identified. Western blot analysis showed that the serum expression level of HP-2 in patient was significantly higher than that in healthy subject, but was not associated with the stage of the Lung squamous carcinoma. Immunohistochemistry showed that the expression level of HP-2 in the lung squamous carcinoma tissues was higher than that in the adjacent normal bronchial epithelial tissues.The results indicated that HP-2 is the candidate biomarker for lung squamous carcinoma, and the serum level of HP-2 might be useful for diagnosis of lung squamous carcinoma. Up-regulated expression of HP-2 in the lung squamous carcinoma tissues may contribute to the high serum level of HP-2 in the patients.

Objective:To detect the expressive levels of hepatocyte growth factor and c-Met and D2-40 in gastric carcinoma;To detect the MLD and its correlation with clinicopathological factors in gastric carcinoma;To investigate the possible mechanism of lymph node metastasis in gastric carcinoma.Methods:Immunohistochemistry method was used to detect the expression of HGF and c-Met and D2-40 in gastric cancer tissue and the normal tissues near the tumor borderline,with microlymphatic density (MLD)counted.Results(1).HGF and c-Met was expressed mainly in cytoplasm of gastric carcinoma cells.The expressive rate of HGF and c-Met in gastric cancer were 67.3%and 78.8%,respectively.(2).The expression of HGF had the positive correlation with histologic types,infiltration depth,clinical stage and lymph node metastasis(P<0.05).No correlation was found in age,tumor diameter(P>0.05).(3).The expression of c-Met had the positive correlation with histologic types and lymph node metastasis(p<0.05).No correlation was found in age,tumor diameter,infiltration depth and clinical stage(P>0.05).(4).The MLD in the gastric cancer was higher than that in the normal tissues near the tumor borderline(P<0.05).The MLD had no correlation with infiltration depth and clinical stage(P>0.05).The positive correlation was found in histologic types and lymph node metastasis(p<0.05)(5).In 52 cases of gastric cancer,there were 31 cases where the expression of HGF and c-Met was positive simultaneously.The simultaneously negative was 7 cases.Correlation coefficient showed that HGF had a positive correlation with c-Met in gastric cancer(r=0.342, P<0.05).(6).In gastric cancer,The MLD in the HGF-positive group was higher than that in the HGF-negative group(P<0.05)Conclusion(1).HGF and c-Met are expressed in plasma in most gastric cancer cell.(2).The expression of HGF has correlation with histologic types, infiltration depth,clinical stage and lymph node metastasis in the gastric carcinoma.The expression of c-Met has correlation with histologic types and lymph node metastasis in the gastric carcinoma.(3).The MLD in the gastric cancer was higher than that in the normal tissues near the tumor borderline.The MLD has the positive correlation with histologic types and lymph node metastasis.(4).The MLD in the gastric cancer has correlation with the expression of HGF,but it has no correlation with the expression of c-Met.

After almost dying from Lyme disease, Pamela O'Kane will run, swim and bike in world class competition in Budapest, Hungary.

In the spring of 2006, Pamela O'Kane was a 45-year-old lecturer at Cal State Sacramento, happily training future teachers in the School of Education. Her own kids were grown, and she spent a lot of her non-work hours in athletic pursuits–running, swimming, biking. She often raced with her sisters Colleen and Denise. Life looked good.

Then, weird stuff started happening. She began losing weight and muscle mass. She developed facial pain, jaw problems, difficulty speaking. Then trouble walking, twitching in her face, tingling in her arms and legs. Her doctors variously thought it might be MS, Guillain-Barre, or ALS. They could do nothing to help and her condition continued to deteriorate. By early December, there was doubt she'd live to see Christmas.

Then, O'Kane developed an ear infection, for which a doctor prescribed antibiotics. The medicine cleared up more than her ear problem. Suddenly, her whole body felt better. Within a week, she took part in a 5K race. ("Didn't do very well," she said. "But I crossed the finish line.")

Although her physical progress didn't last, the incident provided an important clue. Her mother and sister researched on the internet. What medical condition mimics MS and ALS, and can be affected by antibiotics? The answer they stumbled upon: Lyme disease.

In early 2007, O'Kane saw a Lyme specialist in the bay area, who began treating her not only for Lyme, but for several additional tick-borne diseases as well. It was not an easy road. There were episodes of muscle seizures, speech difficulties, and generally feeling horrible. There were ups and downs in finding the right medications–some would help for a while, and then seem to lose their effectiveness. She spent time in a hospital, and then needed to re-learn how to walk.

It was during this time she knew she needed a goal to focus on, something to keep her going through the difficult days. One day, she decided to start training to climb Half-Dome, the rocky peak in Yosemite National Park. She kept that image in her mind, as she worked through the hard, hard times of Lyme treatment. In August 2008, she accomplished that goal.

She kept training. In 2008, O'Kane completed her first-ever half-ironman race. In 2009, she climbed Half-dome again, and also competed with Team USA in the Aquathon event in Australia. ("It was the first time I'd ever had a passport. The first time I'd ever left this country.")

This weekend, she's flying to Budapest, Hungary, once again with Team USA, to compete in the ITU Triathlon World Championships. Her category is Women age 45-49. She'll do the Aquathon–a run/swim/run event, and the Sprint/Triathlon–a swim/bike/run event.

(Two other Sacramento woman have qualified for Team USA in different age brackets, O'Kane's sister, Denise DeTrano, and her friend and training partner, Janet Trippet, a teacher at St. Francis High School.)

It would be tempting to assume that O'Kane's health problems are behind her. Surprisingly, that's not true. While Lyme treatment helped immensely,she still must cope with residual neurological difficulties.

"I make involuntary moaning sounds while I'm running–can't help it," she says. "And I often have lots of shaking and trembling. I get strange looks from the other athletes. But you know what? That's their problem, not mine."

O'Kane has a message for other Lyme patients: "You can fight this disease. It's hard, but you can do it."

O'Kane will compete in Budapest on Sept. 8th & 11th, and then do a bit of sightseeing with her traveling companions. ("None of this put-off-until-tomorrow stuff anymore. I want to travel now!") She's planning to return in time to join the Sacramento Lymewalk on Sept. 25. (11 a.m., McKinley Park, 601 Alhambra Blvd.)

Discuss endoscopic diagnostic value and clinical pathological character of the early upper gastrointestinal cancer. We also performed conventional endoscopy,narrow band imaging system(NBI), magnifying endoscopy,and some of them chromoendoscopy and ultrasonic endoscope to 215 clinic patients of our hospital from October 2006 to January 2007,for the purpose of approaching NBI acting in early upper gastrointestinal cancer diagnosis.There are three parts in this study,that is:1、Endoscopy and clinical pathology of early esophageal cancerObjective: Statistically analyze the result of 55 patients'preoperative endoscopy and postoperative biopsy. Discuss the relation between early esophageal cancer endoscopic character and it's invasion depth.Materials and methods: We collected 55 early esophageal carcinoma that detected by preoperative Endoscopy and comfirmed by post-operation biopsy in our hospital from january 2000 to june 2006,of which were 37 male , 18 female with ages ranged from 37 to 77, mean 57.9. 18 of them were performed preoperative ultrasonic endoscopy.and we analyzed the relation between it's endoscopy character and invasion depth as well as lymphoid node metastasis.Result: Surface diffusion auxotype,intracavitary auxotype (height<5mm)、bilateral auxotype (height < 2mm)、inner wall auxotype(depth < 0.5mm) imply intramucosal carcinoma. intracavitary auxotype(height≥5mm), bilateral auxotype(height≥2mm) ,inner wall auxotype(depth≥0.5mm) and mixing auxotype imply submucosal carcinoma.. There are reverse relationship between histological differentiation and invasion depth of the tumor . Only one was mucosal carcinoma among 4 lymph node metastas and 2 lymphovascular invaded,The other were submucosal carcinoma, All carcinoma in situ wasn't metastasis. By compared preoperative endoscopic ultrasonography performed to 18 patients with postoperative,16 of them are completely accurate. The accuracy is 88.9%.Conclusion: Endoscopic character of th early esophageal cancer was related closely to invasion depth . Detection of endoscope auxotype can help detect it's invasion depth. Endoscopic ultrasonography has important meaning in diagnosing cancer invasion depth. More serious malignant tumor suggests sites of deeper invasion.Deeper invasion cues to more lymphonode metastasis.2、Endoscopic diagnosis and pathology of early gastric cancer.Objective: Discuss the diagnosis method of early gastric cancer to reduce its missing diagnosis.Methods: Collected 24 early gastric cancers detected by preoperative endoscopy and comfirmed by post-operation biopsy in our hospital from december 2004 to december 2006,16 are male and 8 female.aged between 30 to 74, average 53.5.Analyzed their endoscopy charactors and clinical pathological data.Result: 17 of the tumor were located in arcus minor ventriculi, 70.8%.8 were submucosa cancer and 16 were intramembrane.Only one lymphonode( 1/28) were metastasis in a totally number of 529 were found. TypeⅠwere found in 2 patients, typeⅡa were found in a patient, typeⅡb were found in 6 patients, typeⅡc were found in 11 patients, typeⅢwere found in 4 patients. 2 patients are well-differentiated adencarcinoma. 8 patients are moderately differentiated adencarcinoma, 13 are poorly differentiated adenocarcinoma. 1 patient is signet-ring cell carcinoma.Conclusion: Early gastric cancer is poor in specific clinical manifestation. The focus of the EGC is in arcus minor ventriculi. And typeⅡc ,mucosa cancer,poorly differentiated adenocarcinoma are mostly in them. Early gastric cancer has a relatively lower lymphonode metastasis rate than advanced gastric cancer.3、The preliminary application of narrow band imaging in diagnosis of upper gastrointestinal cancer and precancerous lesion.Objective: Discuss the value of applying narrow band imaging in diagnosis of upper gastrointestinal cancer and precancerous lesion. To improve accuracy of endoscopy diagnosis.Methods: We performed conventional endoscopy, narrow band imaging(NBI), magnifying endoscopy,and some of them chromoendoscopy or endoscopic ultrasonography to 215 clinical patients of our hospital from October 2006 to January 2007. Following pathology as diagnostic criteria.Result: Magnifying endoscopy can detect intra-epithelial papillary capillary loops of esophagus. The esophagus mucosa looks light blue with NBI. Using magnifying endoscopy with NBI (MENBI) we can clearly observe intraepithelial and deep layer blood vessel of esophagus. We categorized IPCL into 4 types as below: IPCL-TypeI: Commonly in upper esophagus. IPCL-TypeII:Commonly in middle esophagus. IPCL-TypeIII:Commonly in lower esophagus or inflammation lesion. IPCL-TypeIV: commonly in esophageal carcinoma or epithelium dysplasia.There are 4 subtype of this type: IPCL-TypeIV-1: Commonly in dysplasia and m1 cancer. IPCL-TypeIV-2:Commonly in m2 cancer. IPCL-TypeIV-3: Commonly in m3、sm1 cancer. IPCL-TypeIV-4: Commonly in sm2 and below cancer.Lesions in gastral cavity can be esay observed with conventional endoscopy, While it was diffcult with NBI because its brightness was poor if no magnifying .Pit pattern of gastric mucosa can be revealed with magnifying endoscopy(ME) , And it can be displayed more clearly and stereoscopical with MENBI than ME. Observate on normal gastric mucosa with MENBI, regular small-round pit openings patterns in the fundus small oval shape patterns in the corpus, and,Rod or Strip patterns in the antrum. In the central part of mild atrophic areas gastric pits were found to be decreased in quantity , stereoscopical and color;Mucosa with intestinal metaplasia showed round or oval shape pit pattern, lucency around the gastric pits. Mucosa of dysplasia showed different size and shape pits, in which expansion blood vessels were seen. Mucosa of early carcinoma showed: 1. different size and irregular shape in pit pattern, 2. and disappearing with pit pattern , existing twist and irregular tumor vessels;3. the architecture of pit pattern is distorted. and internal of it was thicker diameters.Conclusion Search the lesions of esophagus mucosa with NBI is easier than conventional endoscopy , MENBI may demonstrate the character , extent and invasion depth of the early esophagus cancer. NBI is not fit to search for lesion in stomach.
